It can't be the UE, it has no idea what hospitalized!Lucia looks like, as it's trapped within dream! part of the construct. Unless it's how Real life!Lucia looks like this, which begs the question why would it know anything happenned at all, I mean, there's no reason for Ira to be gone right now other than some trauma on Lucia's side, Bambino's always here. For it to be the Ue he'd have to know:

1) that lucia just woke and is prime time to impersonate her (which construct!alice supposedly doesn't know, and while we can guess/asume daydream does know, UE is from out of construct, so unless he's reading on spoilers or daydream -currently with emaily- somehow communicated it to him, he wouldn't know)
2) that she got a new fashion upgrade
3) gotten rid of Ira in some way to successfully pull this off (Ira, who last time we saw her was with reggie so if anything reggie would at least have seen what happened to her??)
4) how to get to LOMAM, when we've last seen it in LOHAM

Well,
1) UE can listen in on people a bit. We don't know to what extend but UE has some control over monsters, so it stands to reason that he knows what's going on around them. Lucia was surrounded by monsters when derrick woke up, so all she had to do was drop something about the big event that was coming up in her life and UE would know.
3)Ira's pasty enough that UE could've simply had some monster punch her to dream level 2. Or did it himself. Heck, he could've gotten her away with a ruse and by pretending to be one of the kids.
4)Bane has been watching Alice in locad because people were afraid of her getting impersonated by UE. Moreover, UE started out in derricks poster and managed to end up in loham. It's safe to assume he has some way of traveling between realms.

I don't have an explanation for why UE would know she changed looks. Perhaps he used vanity's trick to look into the real world? Or perhaps he just automatically has updated looks when he goes pretending to be one of the kids.

Either way, I still think calling reggie is the best option here, regardless of weither lucia's real.

If Malice can POTENTIALLY get into the control room of the construct, I'm going to assume UE can, too - and everything lesser. Also, the 'real world' is still part of the construct, and the construct itself helps serve as the UE's prison as far as we know, let's not put it out of possibility that it can pass into the real world if it so chose. It's clearly intelligent and seems to have some kind of agenda.

That said, calling Reggie in is still the best option. So seconding Panur.

Yeah, we need to comfort her in any case, and have Reggie check her. If this Lucia is the real one and she questions why Derrick is having Reggie check her, Derrick can simply say that he’s curious and wanted to see if her appearance was the only thing that changed or not, or something similar and go from there.

Derrick has learned "My scaredy-bat"! At any time Derrick can console Lucia and siphon her fear as his own, however this grants him bravery giving him double attack and HP for 3 turns.

If this is the real Lucia, she should be able to remember the details between when she was at the hospital to when she got home. If it’s ira (or someone else) she likely wouldn’t remember. Asking her to enter the fort would show us ON THE VERY SLIGHT OFF CHANCE THAT PROBABLY ISNT A DANGER AT ALL if this is actually the UE mimicking Lucia. Plus, the fort is a much cozier and safer place.

Also now we have an excuse to abuse Lucia’s easily gained fear by spooking her and siphoning power.
