The River Troll took two sets of damage because Mirdini's potion effected 2 tiles that the River Troll was standing on. Yes there is a way to technically hit them 4 times with 1 potion and kill them in 1 shot.

Mirdini still has 4 HP because at the end of battle you're healed up to full.

River Troll is technically standing on a blue square but since the majority of them are on white squares the +1 defense/-1 attack doesn't apply.
